- To search for a covered provider, click here, or
- Call Customer Service at 877-519-8839, Monday-Friday from 8:00 a.m. to 5:00 p.m. Pacific Time.
- Remember, you never need a referral.
- You can change providers at any time.
- Call the provider directly to schedule an appointment.
- Your provider will verify your coverage:
- Complete any health history forms required by the provider.
- At the time of service, pay your $5 co-payment.

If there is a provider you would like to see who is not currently listed on our Provider Panel and you would like us to contact them, please fill out the Provider Nomination form and fax or mail it to us so that we can contact their office.
If you would like to appeal a visit or service that you feel has been denied in error, please fill out the Adverse Benefit Determination form and fax or mail it to us so that we can have the claim independently reviewed.
